Bell Avenue Nursing and Rehabilitation, located in Elk City, Oklahoma, recently changed ownership and became a Diakonos Group community. With over 350 years of combined clinical, managerial, financial and consulting experience, Diakonos Group, boasts an experienced, knowledgeable and highly capable health care management team that transforms acquired facilities. The acquisition will include an all – encompassing transformation including physical plant renovations as well as a change in leadership. Elk City native, Sean Phanthavong will be serving as Executive Director of Bell Avenue. Sean brings compassion, clinical excellence and unsurpassed leadership to Bell Avenue.
